Comparison of The Effectiveness Between Cognitive- Behavior Therapy and Acceptance and Commitment Therapy on Resiliency and physical symptoms in patients with Ulcerative Colitis.

Background and Aim:
Ulcerative colitis, as form of inflammatory bowel disease (IBD), significantly affects patients’ quality of life and psychological well-being. Given the important role of psychological factors in disease management, this study aimed to evaluate the effectiveness of two psychotherapeutic interventions—Acceptance and Commitment Therapy (ACT) and Cognitive Behavioral Therapy (CBT)—on resilience and physical symptoms in patients with ulcerative colitis.
Methods:
This quasi-experimental study was conducted using a three-group pretest-posttest design with a one-and-a-half-month follow-up. A total of 44 patients with ulcerative colitis referred to specialized gastroenterology clinics in 2021 were recruited based on inclusion and exclusion criteria and randomly allocated to ACT (n=16), CBT (n=12), and control (n=16) groups. Instruments included the General Health Questionnaire (GHQ-28), the Connor-Davidson Resilience Scale (CD-RISC), and the Simple Clinical Colitis Activity Index (SCCAI). Both intervention groups received eight weekly online group therapy sessions, while the control group received routine medical care. Data were analyzed using appropriate statistical tests.
Results:
Results demonstrated that ACT significantly improved physical symptoms and resiliency compared to the control group and difference remained stable during the follow-up stage as well. No significant change was observed in the control and CBT groups.
Conclusion:
Acceptance and Commitment Therapy appears to be effective in alleviating physical symptoms and resiliency in patients with ulcerative colitis. These findings emphasize that integrating psychological interventions with medical treatments can play an important role in managing the disease in these patients.
